Our chronic disease nurse, in collaboration with your doctor, will conduct the diabetes evaluation. This evaluation will include:
- Evaluating lifestyle-related risk factors
- Measuring Body Mass Index, blood pressure, and overall health
- Conducting a physical examination
- Reviewing family medical history.
