CHRONIC MEDICAL CONDITIONS
A chronic medical condition is one that persists for at least six months or longer, such as:
- Asthma
- Cancer
- Heart disease
- Diabetes
- Musculoskeletal issues
- Stroke

CHRONIC DISEASE MANAGEMENT PLANS
General practitioners create chronic disease management plans for patients with complex conditions requiring continuous care and a structured approach.
A GP Management Plan (GPMP) can assist individuals with chronic medical conditions by providing an organized approach to care. It's an agreed-upon plan with your GP that:
- Identifies your health and care needs
- Specifies the services your GP will provide
- Lists actions you can take to manage your condition

TEAM CARE ARRANGEMENTS
If you have a chronic medical condition and complex care needs necessitating multidisciplinary care, we may develop Team Care Arrangements (TCAs). These help coordinate your care effectively from your GP and other health or care providers.
TCAs involve your GP collaborating with at least two other health or care providers for ongoing treatment or services. Inform your GP or nurse if there are aspects of your care you don't want discussed with other healthcare providers.

REGULAR REVIEWS
Once a plan is established, we should periodically review it. This is a crucial part of the planning cycle where you and your GP ensure your goals are met and agree on any necessary changes.
